Avrim Fishkind, M.D.
President and Chief Executive Officer
Dr. Fishkind is a past president of the American Association for Emergency Psychiatry. He has designed multiple comprehensive psychiatric emergency programs including Psychiatric Emergency Rooms, 23 Hour Observation Units, Mobile Crisis Outreach Teams, Crisis Residential and Stabilization Units, Crisis Hotlines, and short-term Crisis Counseling Units. Dr. Fishkind has been employed in the delivery and development of comprehensive psychiatric emergency services in many locations, including New York City, Austin, Texas, Houston, Texas, Washington, D.C, and Toronto, Canada. Dr. Fishkind implemented the psychiatric emergency services at the Houston Astrodome when 25,000 refugees left New Orleans after Hurricane Katrina, mobilizing the largest ever psychiatric emergency response to a disaster.
In 2005, Dr. Fishkind led the clinical workgroup for crisis services redesign for the state of Texas, a more than $200 million dollar infusion to build a new psychiatric emergency system across the state of Texas. In 2007, he formed JSA Health Telepsychiatry to help fill critical gaps in psychiatric services via telemedicine.
Dr. Fishkind completed undergraduate studies in Zoology and Art History at the University of Maryland. He attended Johns Hopkins Medical School in Baltimore, Maryland, and completed his residency in psychiatry at Mt. Sinai Medical Center in New York City. He completed Public and Administrative Psychiatry fellowships at Mt. Sinai Medical Center and Columbia-Presbyterian Medical Center in New York City. He is Board Certified by the American Board of Psychiatry and Neurology. Dr. Fishkind studied furniture design and construction, lithography, and papermaking at the Corcoran School of Art in Washington, D.C. He is a licensed pilot, furniture builder, bagpiper, devoted husband, and proud father of 10-year-old twins.
Mr. Shawn Ball
Chief Operating Officer and Chief Financial Officer
Mr. Ball is responsible for the day to day management and operational direction, strategic planning and meeting the financial objectives of the company. Prior to JSA Health Shawn created Ball Business Consulting, LLC. The company provides Chief Financial Officer, Treasury, and Operations Management services to companies that are in early to mid-stage development. Shawn helped raise over $30 million in equity funding for clients. His client representation is broad covering Technology, Software, IT and Health Care industries. Shawn will continue to serve as Chairman of Ball Business Consulting, LLC.
Shawn began his career at Motorola Semiconductor in Austin, Texas. His 20 plus year career experience included time as an auditor, cost analyst, business analysis, financial planning and analysis, mergers and acquisitions and various operational roles. As Motorola Semiconductor was spun out from Motorola to become Freescale Semiconductor, Shawn worked directly with the investment banking team building financial models for the new company during the IPO process. Additionally, Shawn led the internal controls team that established the Sarbanes-Oxley processes, certifications and governance controls for the company. Shawn worked directly with the Freescale Board Audit Committee in the creation of the corporate audit team of the new company. Later, Shawn gained experience in Freescale’s Investor Relations organization. After Freescale was taken private during the largest technology LBO at the time, Shawn became the Chief Financial Officer of the Cellular business group, a $2 billion revenue business with over 2,000 engineers around the world.
Shawn later accepted an opportunity to join Dell and lead the finance organizations for the Storage business ($2 billion in revenue), Servers ($10 billion in revenue) and Enterprise Solutions Financial Planning and Analysis. Shawn also worked on numerous transactions related to the growth of the Enterprise Solutions business at Dell.
Shawn received his Bachelor of Business Administration degree in Accounting from Texas A&M University and a Masters of Business Administration (MBA) from Baylor University. Shawn is also Certified Public Accountant and a Certified Internal Auditor.
Shawn serves on the Board of Directors of Communities in Schools and CASA of Travis County. He recently relocated from Austin, Texas and now lives in Houston with his wife and two sons.
Gonzalo Perez-Garcia, M.D.
Chief Medical Officer – Texas
Dr. Perez- Garcia was an early member of the UTPA/BCM Premedical Honors College, an 8 year B.S./M.D. This program was designed to generate more doctors from the southernmost 13 counties of Texas, an area that is underserved medically. Through telepsychiatry at JSA Health, Dr. Perez-Garcia has been able to provide mental healthcare for patients in this area including Crystal City, Corpus Christi, Eagle Pass and Harlingen.
He previously worked as an Assistant Professor at the University of Texas Southwestern Medical Center with the Consult-Liaison service of Parkland Hospital, St. Paul University Hospital and Zale-Lipshy University Hospital. In this position, he was also active in the education of medical students and residents. Dr. Perez-Garcia has a Bachelor of Science in Chemistry from the University of Texas – Pan American with minors in Biology and Philosophy. He then attended medical school at Baylor College of Medicine in Houston, Texas where he was president of his medical school class. He continued his studies at Baylor College of Medicine for his residency in Psychiatry where he served as Chief Resident during his final year. He is Board Certified by the American Board of Psychiatry and Neurology. In his spare time, he enjoys cooking, collecting LPs, reading classic American literature and a good crossword puzzle.
Flavio Casoy, M.D.
Chief Medical Officer – California
Dr. Casoy completed his undergraduate and medical degrees at Brown University in Providence, RI where he concentrated on community health. He completed his psychiatry residency at the University of California, San Francisco and served as Chief Resident at San Francisco General Hospital. He also completed a fellowship in Public Psychiatry at Columbia University/New York State Psychiatric Institute.
He is board-certified by the American Board of Psychiatry and Neurology. He has broad ranging experience as an emergency psychiatrist, and most recently led one of the largest state psychiatric hospitals in the United States … where he ran a 14 unit inpatient service. He has a small private practice where he primarily sees patients for psychotherapy.
Dr. Casoy’s primary mission is to ensure that everyone has access to quality mental health care, including, and particularly, individuals with serious mental illness. He is excited to explore the possibilities of telepsychiatry in bringing expert consultants to remote regions or to agencies that would otherwise not be able to afford a psychiatrist. He believes that quality care is rooted in clinical expertise, kindness towards the patient, deep humility about the limitations of our field and systems, and commitment to overcoming these limitations. He is originally from São Paulo, Brazil and is a strong practitioner of cultural formulation as a key feature in behavioral healthcare.
Mr. David Lieske
VP Finance Corporate Controller
As a Native Houstonian, D. David Lieske III, MBA, ACHE, is Chief Financial Officer of JSA Health, LLC. JSA Health is a Houston, Texas based company providing specialty mental health services via telemedicine. Mr. Lieske previously served as a senior healthcare executive with Disability Benefits, LLC and Diabetes America, Inc, providing Operations, Management, and Business Development services. He has fifteen years’ experience in planning and implementing progressive healthcare delivery models and services; eight of those years working in the Texas Medical Center for Baylor College of Medicine and Texas Children’s Hospital.
Mr. Lieske received his MBA in Finance from the University of St. Thomas in Houston, Texas, and a BBA in Marketing from Texas A&M University in College Station, Texas. He is an active member of the American College of Healthcare Executives and a past member of the Medical Group Management Association, 2000-2006.
Mr. Lieske attended the Jesse H Jones Graduate School of Management at Rice University in Houston, Texas, in 2004 and received a Certificate of Medical and Healthcare Management. The program was a partnership between Baylor College of Medicine and Rice University to assist in developing focused executive leadership skills across a wide array of healthcare reform topics.
Chief Quality Officer
Liz joined JSA with a background in healthcare operations, service development, and compliance.
In her previous role, she initiated patient oriented programs and services, developing care models for a diabetes specific provider group that grew to a multi site organization serving major metropolitan markets in Texas and Arizona. The thirteen centers were Recognized Education programs by the American Diabetes Association, one of two bodies providing program certification for federal providers. Liz was also instrumental in securing NCQA Diabetes Recognized Provider status for the physicians in the affiliated Physician Association. She established a Compliance Committee and implemented a corporate compliance plan and audit tools for achieving ongoing improvement in center operations. Prior to that she owned her own business and had several employees, providing management and diabetes services to a Houston hospital as well as related diabetes consulting services in the community. She has a clinical background as a Registered Dietitian and Certified Diabetes Educator.
Liz is committed to high quality and high levels of performance, delivering essential outcomes for all customers, including patients, physicians, and health systems.
Liz has a BS degree in Nutritional Sciences from Cornell University and over 15 years experience in healthcare development and administration.
Terrie Mayfield Clinical
Terrie Mayfield, M.P.A., is Clinical Operations Manager. She is responsible for management of the daily operations and the physicians of the clinical practice. She serves as the direct point of contact for both clients and staff and monitoring, planning and further developing the infrastructure of clinical operations, as well as the electronic medical records.
Her previous experience includes directing operations in a wide range of both medical and psychiatric settings. Her career started in 1982 with Vernon State Hospital, but includes home health, hospital and practice management. She has directed three physician groups, including two managed care/marketing partnerships. As the Administrative Director of a hospital, she developed and managed multiple programs, including a Psychiatric Partial Hospitalization Program. Most recently, she was with the Mental Health Emergency Center in Lufkin, Texas.
Terrie studied Psychology at Texas Tech University and has a Bachelor’s Degree from Midwestern State University. She has a Masters Degree in Public Administration from Stephen F. Austin State University. She is the mother of Cameron, a student at Colorado Mountain College and Lacy, a Diagnostician for the Lufkin School District. Lacy has two daughters, Lila Marie and Emma Grace.
LaShandra Bradford came to JSA with over 18 years experience in medical program management. Amongst her positions, Ms. Bradford worked as the Executive Assistant with MHMRA of Harris County-Comprehensive Psychiatric Emergency Program, where she learned the inner workings of an urban psychiatric emergency service. At MHRA she was involved in the implementation of a Crisis Stabilization Unit, Crisis Residential Unit, Mobile Crisis Outreach Team, and Crisis Counseling Service.
Ms. Bradford then moved to be Senior Assistant for Chairman of Leukemia with M.D. Anderson Cancer Center where she assisted with all aspects of department administration. She joined JSA Health Telepsychiatry in 2010 reshaping all daily clinical operations when she started with JSA…now having moved to working as project manager for all new client telemedicine implementations.
LaShandra is the mother of daughter in college majoring in psychology and a son serving in the U.S. Navy. She is a proud grandmother to her 4 year old granddaughter.
Information Technology Director
William Yancy, MCSE is Information Technology Director of JSA Health Telepsychiatry. Mr. Yancy previously served as Lead IT Consultant for Technologies Global, where he provided supervisory and technical oversight for core services. He has been instrumental in the planning and coordinating the development, implementation, and deployment operations for information systems and technology for business needs. Mr. Yancy has worked 12 years in IT with MHMRA of Harris County. He was key in the rollout , setup, and maintenance of the agencies video-conferencing. He also served as Level III support for 24 agency locations and worked on many agency wide IT projects.
William attended the University of Oklahoma. He completed his undergraduate studies towards a Bachelor of Science in Management Information Systems from Houston Baptist University. In 2001, Mr Yancy completed the Network Technologies program at Southern Methodist University, School of Engineering and is Microsoft and Comptia certified in many areas,
Rasheeda has 4 years of healthcare staffing and operations management experience. She has successfully recruited and staffed physicians in multiple specialties including: Emergency and Hospital Medicine, Psychiatry, Psychology, Dental, Neurology etc. Rasheeda’s background also includes working as a scheduling coordinator, provider liaison, training and credentialing physicians. During her tenure with a Veteran’s Administration contracting company she managed the recruitment for the busiest multi-state region through training, coaching and guiding a team of recruiters. She has also worked in a locum tenens agency and a contract management company.
Rasheeda completed an undergraduate degree in Information Technology in 2009 from the University of Phoenix. She is working toward an MBA with Keller Graduate School of Management.
Ray St. Germain
Director of Business Development
Mr. St. Germain previously served in a number of clinical and management roles with Psychiatric Solutions, Universal Health Services, Aurora Behavioral, and SUN Behavioral of Houston. He has extensive operational and clinical experience specializing in program development and high risk assessment in the mental health field. He has practiced as a Licensed Social Worker since 1998. Mr. St. Germain is a fourth generation Houstonian and life long Texas resident. He attended Texas Christian University where he received his Bachelor of Arts in English/Government and the University of Houston Graduate School of Social Work for his MSW. Ray is an avid history buff and enjoys tennis, golf and is most at peace riding his bike in the beautiful mountains of our Rockies.
Whitney graduated from the University of Houston, Cum Laude, in May of 2010 with a BA in Creative Writing and a minor in Studio Art. She joined JSA part-time in September of 2012 and transitioned to full-time in May of 2013. She currently works part-time for Spec’s Liquor, having been with the company since 2007, and dabbles in freelance illustration and photography on the side.
Ms. Franklin joins JSA Health Telepsychiatry to expand our credentialing team, and brings a variety of experiences and her enthusiasm to the role. Dorothy is a native of California and moved to Texas in 2009. She graduated from Cal State Hayward in 2000 with a BA degree in Human Development. Dorothy later returned to school, earning a MS degree in psychology from Capella University.
Ms. Franklin worked for Kaiser Permanente as a Credentialing Administrator in California before relocating to Texas in 2009. From 2011 to 2016, Dorothy worked for HCA Shared Services as a credentialing verifier/specialist. She now joins the JSA Health team as a credentialing specialist. She is the mother of two sons and enjoys gardening, cooking, reading and watching a good horror movie.
Donna Lake joins JSA Health Telepsychiatry as part of our credentialing team, our Medical Staff Licensing and Credentialing Specialist. She has varied experience in credentialing over her professional career in medical staff services with hospitals as well as large and small credentialing organizations.
She provides support to all of our physicians in acquiring licenses for states where the physician is not currently licensed. Donna is from Galveston originally, and has lived in the Houston area throughout her life. She is enthusiastic about supporting JSA’s growth and working with the doctors to provide services in new areas of the country.
Myra Coronado was born and raised in Houston Texas. In 1997, she completed her culinary training which included Restaurant and Hotel Management. She accidentally fell into the world of Healthcare after a few months of being in food service taking on an information services position while working at St. Joseph Hospital. Myra went on to work with Doctors Hospital Parkway-Tidwell and quickly moved into the seat of the CBO Medical Insurance Biller.
Later taking a position at Triumph Healthcare LTAC for four years as Corporate CBO Billing Coordinator where she led the billing team into a 22 facility expansion across the united states and, in the last five years, with The Center For Hearing And Speech as their Admissions/Billing Coordinator where she handled enrollments for The Melinda Webb School. She strives for good health and continuously works toward her personal goals.
