JSA Health Telepsychiatry’s Mental Health Emergency Center Design for Austin, Texas Breaks Ground

JSA Health Telepsychiatry is pleased to announce the latest version of it’s award winning Mental Health Emergency Center (MHEC) design. The new MHEC, to be built in Austin, Texas, was funded by the St. David’s Foundation who provided $8.9 million dollars to build and operate the new Psychiatric Emergency Room, Extended Observation Unit, and Crisis Residential Unit that will be built just east of downtown Austin.
JSA’s CEO Dr. Avrim Fishkind will be present on this project throughout the design and implementation process. The St. David’s Foundation turned to Dr. Fishkind in late 2013 to help them determine what design for psychiatric crisis services was needed in Austin, Texas. Dr. Fishkind proposed the design and helped the St. David’s Foundation Board to determine that the $8.9 million dollars was the right price, for the right service, in the right place at the right time … the mantra of this new era of accountable care.
Dr. Fishkind and the JSA team have been assisting Austin Travis County Integral Care, the community mental health authority that will run the new MHEC style center. JSA has provided consulting regarding physical plant design, clinical operations and procedures, medical and psychosocial service delivery, staffing and budget issues, and implementation of telepsychiatry. When the facility opens, JSA Health Telepsychiatry will be providing the on-demand emergency psychiatric evaluations and make make tele-rounds at the Austin MHEC.

A long-awaited Austin facility for people suffering a mental health crisis is moving from the drawing board to an early April groundbreaking, with an opening planned for early next year, officials will announce Tuesday.

The Judge Guy Herman Center for Mental Health Crisis Care, at 6600 Ben White Blvd., will stabilize people in a calmer, more appropriate and less expensive environment than an emergency room, a psychiatric hospital or a jail. It is being named for a Travis County probate judge who has long advocated expanding mental health services.

Officials said in July 2014 that they hoped to open the center in 2015, but the process has taken longer than anticipated, said Ellen Richards, chief strategy officer for Austin Travis County Integral Care, the public mental health organization.

“We’ve been going through design changes so it meets the needs of consumers and families,” she said. “And in a boom town, getting a new building designed and built is time-consuming.”

Herman, who commits people to the Austin State Hospital, said he was honored by the naming and wasn’t concerned about the delay.

“It’s better to go slower and get it right rather than go faster and get it wrong,” he said, adding that the center will “absolutely” reduce the 3,400 state hospital commitments he makes each year.

Plans for services at the Herman Center also have evolved.

Its 42 employees will provide patients with holistic, integrated care, said Sherry Blyth, director of crisis services for Integral Care. That means not only will patients get intensive psychiatric services 24 hours a day, but those who need primary medical care or social services will be linked to that help, Blyth said. Patients will stay an average of three to five days and then go home or to a hospital if needed.

“We would have assured that they’re firmly established with ongoing care” for up to 90 days, she said.

Since 2005, such a center has been discussed among mental health officials, law enforcement officers and Central Health, Travis County’s hospital district. Almost two years ago, the St. David’s Foundation said it would provide an $8.9 million grant to Integral Care to build and operate the facility for two years. After that it will be self-sustaining.

“I’m just thrilled that it’s opening and it will be the first center of its kind” in Austin, said Earl Maxwell, the foundation’s CEO.

Construction of the 12,000-square-foot, 16-bed facility will cost $4.3 million, Richards said. It will be adjacent to Central Health’s Southeast Health and Wellness Center, and Central Health will lease the land for $1 a year.

Although psychiatric emergency services became available at University Medical Center Brackenridge two years ago, that ER can’t hold patients for days. It can put patients in restraints and seclusion, but that won’t be done at the Herman Center, officials said.

“For people in a psychiatric crisis, it reduces the feeling of being stigmatized for having needed emergency psychiatric care,” Blyth said. “We will be able to admit people directly from the community without them having to go to an emergency department.”

Retired educator Valerie Dodd Milburn of Austin, who is 55 and has bipolar disorder, said she welcomes the center. She experienced a crisis in the 1990s and was sent from the ER to a psychiatric hospital “not because I needed the environment but because it was the only place available,” she said.

“That situation still continues,” added Milburn, who chairs Communities for Recovery and is active with the Austin chapter of the National Alliance on Mental Illness.

Officials expect the center to serve about 1,460 patients in the first year and 1,655 in the second year.

Here in Texas, the state government in Austin developed an excellent program known as TCOOMMI -Texas Correctional Office on Offenders with Medical or Mental Impairments. It’s a very progressive program designed to help such offenders re-integrate into the community and avoid future incarceration and hospitalization.

JSA is now serving TCOOMMI clients at Heart of Texas MHMR in Waco … the Burke Center in Lufkin …  and Community HealthCore in east Texas. These programs presents a unique opportunity for JSA psychiatrists to impact an important population to increase community tenure and functioning. TCOOMMI identifies and responds to statutory procedural or regulatory practices that impacts offenders with special needs. Examples of activities include:

  • Coordinating with the Texas Commission on Jail Standards on strategies to improve the screening, identification and treatment of inmates with mental illnesses in county jails.
  • Establishing a process for ensuring continuity of care for defendants being returned to jail after a determination of competency. This includes post-release medication reimbursement and support services after sentencing.
  • Developing a process to evaluate competency evaluations’ compliance to statutory provisions set forth in 46.B, Code of Criminal Procedure.
  • Implementing and monitoring the memoranda of understanding required between local and state criminal justice and health and human service agencies to ensure a continuum of care for offenders with special needs.

Case Management for Adults

Lavaca Medical Center is a 25-bed, rural, 86,409 square foot, critical access hospital.  LMC serves a target population of approximately 19,347 people. The service area includes the towns of Hallettsville, Shiner, Moulton, and Sheridan, Flatonia, Sweet Home, Schulenburg, and Yoakum. The facility has been in operation since 1977, and has undergone several renovations, with the last being completed in 2013. Lavaca Medical Center is designated as a Level IV Trauma Facility. The Alton C. Allen Emergency Department at LMC has 3 regular ER beds and 2 separate Trauma Rooms. JSA Health Telepsychiatry … an early pioneer of the use of telepsychiatry for university based student health services, has been chosen by Sam Houston State University of Huntsville, Texas to provide psychiatric evaluations and medication management to Sam Houston students. JSA has long recognized that most university and college student health centers are unable to provide psychiatric services due to shortages of available psychiatrists interested in student mental health. Telepsychiatry makes it easy to link interested universities who wish to provide their own psychiatric services with qualified student health psychiatrists. Many universities remain dissatisfied with having to simply refer students to available psychiatrists in the community … especially when services may be needed emergently or for crisis situations. Emergency telepsychiatry evaluations … done within minutes of a request … provide universities the peace of mind needed to deal with potentially suicidal or homicidal students, or students with extreme anxiety, depression and other mental health issues.
JSA will also be available to Sam Houston State University students for ongoing medication management services. JSA is very pleased to be affiliated with such an esteemed campus … and we look forward to helping the already superb counselors, psychologists, nurses and other mental health professionals practicing at their Student Health Center.

Sam Houston State University, located in Huntsville, Texas, is a member of The Texas State University System. The school was created by the Texas Legislature in 1879 as Sam Houston Normal Institute to educate teachers for the public schools of Texas. The baccalaureate degree was first awarded in 1919.

In 1923, the institution’s name was changed to Sam Houston State Teachers College. Two years later, the college was admitted to membership in the Southern Association of Colleges and Schools (SACS) as an accredited institution of higher learning. A graduate degree was authorized in 1936, and the curriculum was expanded to emphasize preparation in a variety of fields.

Following World War II, an increase in students and faculty as well as a wide range of faculty-research activities provided impetus for the continued emergence of a multi-purpose institution. In recognition of these developments, the institution’s name was changed by the Texas Legislature to Sam Houston State College in 1965. The number of graduate degrees conferred increased significantly in the late 1960s; and the Texas Legislature, recognizing the changes that had taken place, changed the name of the institution to Sam Houston State University in 1969.

In the 1970s, the University was granted permission to offer its first doctorate, a Ph.D. in criminal justice. This program grew to be one of the largest and most recognized doctoral programs in the country. In the 1980s and 1990s, the University completed a number of academic, athletic, and support facilities. With the improvement of faculty and facilities, the University set a vision to become one of the best regional universities in the country.

During the last five years of the twentieth century, the University expanded its reach by offering programs online, at The Woodlands in a multi-institutional teaching center, and various other off-campus sites. Beginning in 2000, the University expanded its building program and committed resources to develop and maintain nationally-recognized academic support programs. Sam Houston State University increased the number of doctoral programs, including programs in education and psychology, and experienced a tremendous surge in enrollment and name recognition.

Currently Sam Houston State University is organized academically into seven colleges: Sciences, Business Administration, Criminal Justice, Education, Health Sciences, Fine Arts and Mass Communication, and Humanities and Social Sciences. Students are offered an extensive range of bachelor’s and master’s degrees, as well as doctorates in selected areas. The faculty and the University are recognized regionally, nationally, and internationally.

In 2012, Hendrick completed a community health needs assessment which identified the need for more mental health resources.
